Many Possibilities: One thing that can cause this is clenching or grinding the teeth. It can also becaused by tension or stress tensior even migraines and it could be something more. You probably need to have a check up with both your dentist and md to evaluate and see if one of them can determine the cause. If it is frome grinding or clinching that is usually fairly easy to detect with an exam of the teeth.

Maybe infected: You need to have your dentist check your tooth for possible infection. If you do have infection, then you will need to do a root canal.
