Many possibilities: When you ask "what could cause" these symptoms, there are a large number of possibilities. Pain that is associated with motion of the rib cage, as you describe, may come from inflammation, injury or abnormality of ribs or attached cartilage or muscle and includes irritation of the lining around the lung (pleura) which can be associated with pneumonia, blood clots, or other lung trouble.
