67 swine flu vaccine: A vaccine hurriedly issued in ~ 1967 after an outbreak of swine flu at a military facility was associated with a relatively large number of cases of guillian-barre syndrome. The problem was traced back to flaws in vaccine production & lead to more stringent production & quality control standards going forwards.
