2 different things: Most likely cause of the blood on the toilet paper are benign rectal issues like hemmorrids, fissures etc. You should always however have it checked out especially if you are over 40.

Associations: The back pain with radiation and the blood on toilet tissue may or may not (more probable) be related. The blood is most likely to be from hemorrhoids or a small fissure, but should always be followed up so as not to miss something more severe. The back pain, however, should also receive attention as you may have a renal stone, a spinal disk problem, or other issues. See a good pcp.
