Perhaps: I presume you had a kidney transplant in the past and you are on immunosuppressive therapy. A slow rise in creatinine can be seen in several different settings including chronic rejection (=chronic allograft nephropathy or IF/TA), or other settings including BK virus nephropathy. Often renal biopsy will be recommended to see exactly what is going on at the tissue level.
