Treatment needed: The current recommendations regarding TSH levels and pregnancy are for the TSH to be <2.5 in patients seeking fertility. Since normal is usually up to 4.5 this means we are treating patients with TSH levels that are in the normal range. Pregnancy rates could be reduced and miscarriage rates could be increased in patients with hypothyroidism even if the free T4 levels are normal.

Yes: Hypothyroidism can have adverse effects on the pregnancy and the fetus. It's very important to get this treated if pregnancy is planned.
