A reactive change: "intestinal metaplasia" occurs when some other epithelial lining (like esophagus) is changed over time into intestinal epithelium- like found in the intestine. "focal" means this involves a limited area. "without dysplasia" means that although this intestinal epithelium is in the wrong place, it is healthy and not showing changes that are atypical or in danger of turning into a cancer.
