Hard to say: Ischemia means a lack of oxygen to the tissues. This is usually caused by a blockage in the supplying artery. Blockages are most often caused by atherosclerosis which is a slowly progressive disease. Sometimes the plaque (diseased part) can break off and suddenly occlude the vessel. Also the disease can cause symptoms which are ignored until it is too late.

Plaque rupture: Most are progressive narrowing to the point of serious impairment of coronary blood flow with exercize etc. Causing angina or heart attack some are acute thrombus on a plaque or elevation of the plaque like a trap, door- dissection or rupture of the built up area of disease.
