Spurs can occur : Both on the bottom and in the back of the heel. There is a debate if the actual bone causes the pain or not l for the most part the treatment is the same. Surgical treatment ( for a spur on the bottom of the feet)when attempted is often also the same surgery. Some docs will elect to remove the spur as well.

Not necessarily: Many people have heel slurs visible on x-ray but only a tiny fraction of those actually have any pain. It is felt that while a heel spur may contribute to heel pain it is not the sole (sorry for the pun) cause. If you've been told you have a spur because of a recent x-ray but you don't have any pain, then don't worry about it.
