I have sterile pyuria and have tested negative (multiple times) for uti's, kidney disease, sti's, and hiv. what could be causing my sterile pyuria?

Difficult to say: "sterile pyuria" in classic medical literature was associated with either chlamydia or tuberculosis; that being said, the incidence of tuberculosis is much lower now, and in general, we ignore pyuria if there are no symptoms (according to the idsa guidelines). If you are symptomatic, then further evaluation may be needed such as AFB cultures.

Inflammation?: "pyuria", or white blood cells in the urine is a sign of inflammation. The inflammation could be from a number of causes, and infection is just one of them. When you have inflammation from non-infectious causes, you could see "sterile" pyuria. Some of these non-infection related causes include drug-related allergic reactions, autoimmune (lupus), malignancies, etc.
