Depends on technique: There are many different approaches that may involve implants, flaps or grafts. Certainly the collaboration between your surgeon and plastic surgeon will help to minimize the visibility of the scars of both the mastectomy and/or reconstruction.

Yes: All surgery leaves scars. The goal of plastic surgery is to leave fine lines but unfortunately some people scar worse than others despite all precautions.

Scars are permanent: The scars from any surgery including breast reconstruction are permanent. However over time, the scars will most likely fade to a thin white line. Massaging the scar with vitamin E oil or cocoa butter may help speed up the process to make the scars less noticeable.

Depends: If your breasts are youthful looking and your surgeon can preserve your nipple then you could have very little visible scarring. At 22 it sounds like you may have brca issues so again it depends in the above. Shaped implants will give the best result. It is important to remember that even wide scars once mature are often not very noticeable.

Depends on situation: If you are having a cancer surgery, the scarring really depends on what kind of mastectomy is required to treat the cancer. If the tumor is small, and not near the nipple, a "skin-sparing" or "nipple-sparing" mastectomy would preserve more of your tissue than a traditional version. This would be better for you, in terms of few, smaller scars, particularly with the nsm.
