Yes: 458 is a high number. Please see your doctor to get your regimen adjusted. You might need a change in insulin, additional Insulin or addition of an oral medication.

Yes: Your sugars are not optimal, they should be not more than 120. You need to see your doc to adjust the Insulin dose as well as modify your diet.
