Endocrinologist : It sounds like our six year old could be having premature development does she have any other signs of prematurity such as enlargement of vulva, pubic or axillary hair my advice is to see a pediatric endocrinologist who will better evaluate test and eventually treat the child this important because this could become quite serious.

Oh yes. : Breast development can happen at 6, not 9 like previously thought. So a small breast bud under both nipples is normal, and if she's messing with them, like playing with a loose tooth, they can get sore and irritable. If she starts to get excessive hair under the arms or in the vaginal area, that'd be a little early, but breast buds can certainly happen by 6.
