Birth defect: Most people with hiatal hernias are born with them. Being overweight/obese may make the hernia worse; reflux may lead to mild cough or shortness of breath, which can worsen if the reflux leads to repeated episodes of acid going into the lungs (aspiration). There are many other causes of shortness of breath, the most common being out of shape. See a pulmonologist and get checked out.

Hiatal hernia: It can be, but generally only large hiatal hernias cause shortness of breath. Heart work up being negative is good. Now go see a pulmonologist.
