Not necessarily: You do not have to get your gallbladder removed. People with decreased gallbladder ejection fraction (<50%) often have symptoms related to this problem such as pain on the right side after eating, inability to tolerate fatty foods and bloating. > than 80% of people will get better after having thier gallbladder removed. Rarely people have trouble digesting fats after surgery (cramps/ diarrhea).

Some reasons: Gall bladder ejection fraction is related to response of gall bladder to intravenous injection of hormone cck or fatty meal. Abnormally low values can be due to prolonged IV nutrition, eating within 6 hrs prior to study, pain medications, and hepatic disease. The speed of IV injection ofcck, can affect gb ef. Symptoms not gb ef are main criteria for gb disease. People without gb can digest fats.

Maybe and no: If your gallbladder ef is 0%, that means you have gb dysfunction. I you have symptoms then surgery is indicated. Yes you would be able to digest fat. A low fat diet is recommended after surgery for a few weeks but patients do well.
