Win-win: Having the surgery is much less dangerous than not having the surgery, particularly if your obstruction is a complete or nearly complete blockage.

At risk: This is major surgery since it involves opening the abdomen and resecting the obstruction. Maintaining volume during surgery and post operatively is imparitive. If there are no complications during the procedure and if the patient has been cleared from a cardiovascular standpoint, the risk is less. Post op care will be in the ICU until stable. I am assuming sbo and surgery must be done.

Analyse risk/benefit: Chronic kidney disease will increase your risk for any elective or emergency surgical interventions. However, if intestinal obstruction is not responding to medical therapy and your team determines emergent surgery is needed to prevent bowel perforation, the kidney issue will be managed. You may need dialysis temporarily.
