Yes: There are no otc antibiotics that I am aware of for uti; the otc meds are just pain relief, which do not address the underlying infection. I would go to an urgent care or er, yes.

Need antibiotics : A urinary tract infection pregnancy requires treatment with antibiotics. I would contact your ob/gyn today and let the nurse know the situation. They should either see you today or call in antibiotics for you. Waiting for 4 days is not reasonable.
