Ant Disc displace: Sounds like you still have anterior disc displacemnt without reduction. When the arthoscopies were performed, did they perform disc plication and pull the disc back and tie it in place? If they did and it is recurrent, you may need open joint surgery.

A Closed lock TMJ: The disc is locked in front of the condyle of the TMJ and prevents forward advancement of the mandiblar condyle. This is limiting your opening too. It is a chronic condition by your report of one year, therefore not likely treatable only by arthroscopic techniques. Have you had a pull forward spint (farrar splint) and have you been in it 24/7 with manipulation to unlock the TMJ closed lock?

Closed Lock: You have a closed loick. Or as you say - disc displacement wihout reduction. If your symptoms are severe, you may need open TMJ surgery. See a TMJ surgeon with vast experience for advice.
