Anterior disc: Disc plication is an option. Or removal if the disc is damaged too much.

Depends: How long has trouble with joint(s) been going on? Were the arthroscopies done one one joint or one for each? Has the dentist been adjusting the splint? Recapturing an anteriorly displaced disc is not easy if it is long term and posterior ligaments are stretched or torn. Try seeing a dentist who is trained in neuromuscular treatment. Surgery of the joint often causes more scar tissue.

Need more: Information. Without seeing you and discussing what else has been done it would be difficult to discuss what other options are available. If your dentist has no further options and enough time has passed consider finding a tmd specialist through www.Aacfp.Org or www.Aaop.Org in your area.

Open surgery: If you've had excellent conservative treatment, had arthroscopy by a skilled surgeon, and appropriate control of ancillary factors (like clenching and grinding) open surgery may be a solution. See a vastly experienced TMJ surgeon for advice.

See OFP specialist: Orofacial pain patients can be treated with conservative, non-surgical and reversible procedures. Surgical treatment of the disk may be done, with variable success and not recommended. See OFP specialist, they are the experts in this field.
