Stage fright : This is the most common form of social phobia and also treatable. Consult your physician or a psychiatrist, a beta blocker like Propranolol can help you if taken half hour before performance. This can be combined with ssri like Lexapro (escitalopram) too for more severe form of social anxiety. Cognitive behavior therapy has been shown to be helpful.
