What is your opinion: should rai absolutely be given after thyroid removal with a <1cm malignant nodule with hurthle/onocytic cell changes.?

No !!!: Stage one malignancies do not require rai. The benefits do not outweigh the risks. There is no mortality benefit and you will have the added risk of secondary cancers. Rai is reserved for stage iii-iv disease or stage i-ii with high risk features. Hurtle cell is not a higher risk feature.

Not needed: Make sure that you dont have other risk factors that put you in high risk category - only 1 nodule ? Not multiple / clear margins / etc ; if no high risk features , then not indicated.
