Lab measurement: Total testosterone is a lab measure of total concentration in blood, representing "all" testosterone. It includes fractions tightly bound to sex hormone binding globulin (shbg), loosely bound to albumin, and free testosterone (not bound to anything, therefore available to do what you expect of testosterone). Labs use different methods to measure total testosterone so know your reference ranges.
