Supply and demand: Angina is caused by heart muscle not getting enough blood to meet its needs, think supply vs demand. By relaxing smooth muscle in the walls of coronary arteries, nitrates increase blood flow. They also dilate the body's veins, so less blood returns to the heart, and the heart walls are less stretched outwards. This reduces tension on the heart wall muscle, and hence demand for oxygen/blood.
