The bags are sometimes due to fat pads under the eyes that grow as we age and can become quite large. The surgery is often done as an out patient procedure and takes less than two hours. Yes: The "bags" in the eyelids is a result of loss of elasticity plus weaking and stretching of a small fibrous band (septum) which permits the bulges to show due to pressure of intraorbital fat. Swelling is most in am, but as we age the fullness becomes a constant feature. Reduction of some periorbital fat, along with a bit of skin is the usual treatement.

Blepharoplasty: People with bags under eyelids are genetically predisposed. These are fat pockets and are independent from your weight. When surgically removed they do not come back. In younger patients, an internal incision (transconjunctival) is used to remove the bags. If there is alot of skin stretching and wrinkling, the a combined internal and external approach is used.

Multiple ways: As we age, the tissue that holds the normal fat which surrounds our eyes starts to weaken and then bulges outward causing these types of "eye bags". Most of the time surgery is necessary to fully correct this, but there are many over the counter eye creams or anti-puffiness creams which work really well.

Lid Surgery vs. Fill: Depending on the lid anatomy and where in the aging process you are, you might benefit from fat reduction blepharoplasty (an outpatient procedure). This procedure can be combined with (or in certain cases replaced by) a camouflage filler treatment in the "tear trough" (the indentation between the lid and the upper cheek). Some will also benefit from a combined laser resurfacing over the lower lid.

Bad Eyelid Bags: Traditionally this is treated by removal of the fat pads that bulge out from the orbit causing the bags to appear. More recently, concerns with excessive fat removal causing hollowness or gaunt appearance has led many surgeons to advocate fat preservation with correction of the "tear trough" below the bags with fat flaps or fat grafts or cheel lifts. "non-surgical" techniques use injected fillers.

Surgery vs Filler: For most young patients, eyelid surgery for lower lid bags is "going squirrel hunting with an elephant gun". Too much for too little. Usually, either a filler such as juvederm can be injected as a temporary (12-18 month) fix with excellent results. Using your own fat to do this is a second way to treat this with a needle, but the results are, essentially, permanent.
