Your : Your son definitely has a problem. He may have a condition called lichen sclerosis. He should see a urologist or pediatric urologist for evaluation. The problem may be treated with steroid cream. If severe, he may require a circumcision.

Tight foreskin: This is called phimosis. He needs some prescription strength steroid cream to be applied twice daily to tight part of foreskin as he attempts to retract. Alternative would be circumcision or prepucioplasty which is surgical procedure incising thru tight area of skin vertically & suturing it transversely. A urologist can advise.

While : While this could be a hygiene issue, his symptoms are suspicious for a condition called lichen sclerosis...Especially if he is not pulling back the foreskin because it is too tight/painful. If better washing/cleaning does not fix the problem, he should see a urologist for evaluation.
