I : I don't have the advantage of seeing you or your ct scans, but this sounds like a possibility might be buccal space cellulitis. Is there any fever, warmth, pain, or redness to the cheeks? It's odd you haven't responded to an appropriate course of antibiotics. The addition of a short-term course of steroids may help you with the swelling. I would think that consultation with an infectious disease specialist would be appropriate in this setting.

If : If you have not already done so, you should see a board certified oral and maxillofacial surgeon. Six weeks is a long time for an infection to be spreading in the face of antibiotics. From the description of your symptoms, i find it amazing that nothing showed up on the cat scan. An MRI may be in order to evaluate the soft tissues better. This may not be an infection, but some type of soft tissue growth. I would also suggest that you find a regional teaching institution in philadelphia (u of penn, temple, jefferson) to provide you with a second opinion.
