I : I am trying to visualize how large this is. I can't diagnose this. If the area is swollen, tender and warm you could have an infection, possibly even cellulitis. If this is the case, it is recommended that it be evaluated. If the swelling is continuing to spread you may need to be seen at the emergency room. If you develop cellulitis or if you already have it, that can be serious.

Hard : Hard to tell without a clinical examination so i would see a dermatologist. If it was an epidermoid or acne cyst, treatment options would include an antibiotic, drainage, injection of steroid, or possible surgical removal. But first, i would have it evaluated to confirm the diagnosis and discuss treatment options. See a pic of a cyst and treatment options at the link below. I hope that helps!
