Lower : Lower eyelid blepharoplasty definitely improves the appearance of under eye bags. Different approaches are used for this operation. In patients needing treatment of both excessive loose skin and full under eye bags, an incision is made beneath the eye lashes and excessive skin and fat are both removed. In patients who only need the under eye bags and fullness treated and don't have skin excess, an incision may be made inside the eyelid to reduce the under eye fat and not leave external scars. Very puffy, fluid filled under eye bags called "festoons" may occur in older patients, and these are much more difficult to treat than typical under eye bags.

In : In fact the only real treatment for bags under the eyes is blepharoplasty. The creams that you see advertised on tv and in magazines will typically leave you disappointed.

Yes: If the bags are a result of protruding fat pads, then yes, surgery (blepharoplasty) to reduce or reposition the fat will help. Your surgeon should inquire about the presence of environmental allergies that may worsen or be the cause of this condition, in which case, allergy treatments would be advisable first.
