I would expect you to always look better than you would have otherwise (more volume), but the natural aging process will proceed over the years. One aspect of the procedure is that there is some variability in how much fat "takes". In some cases it is beneficial to do a touch up transfer procedure 12 or 18 months after the first to maximize the results.

Fat : Fat that is transferred to another body site and survives will, for all practical purposes, be permanent. However this does not mean the it is a one time operation. You may have to repeat this in order to supplement or compensate for the ongoing aging process which is constantly changing the appearance of your face.

Permanence: The best, most experienced surgeons do two procedures when possible to increase predictability, results, and "permanence". Single procedure fat transfers with predictably good, noticeable long term results will generally leave you being swollen for an extended period of time (few weeks), while more careful, double transfers will not as much. The reason better fat graft survival at 6 weeks: blood!

New techniques of processing the fat has significantly improved the viability of transplanted fat. The fat cells that survive the transplant are living tissue and should act like native fat. Generally, what you see left after 3 months is your final result.
