Yes: Surgery is reserved for large clots that are typically immediately life-threatening. Smaller clots are treated with blood thinners which help prevent the clots from becoming larger and give a chance for them to naturally dissolve.

Yes: Massive blod clots in the lung can be removed by open heart surgery. Chronic debilitating clots may also be removed with open heart surgery. However most clots to the lung can be treated just with blood thinning medication.
