Yes: Radioactive iodine treatment for hyperthyroidism has a 5-10% failure rate. Failure can be either persistent hyperthyroidism more than 6 months after treatment, or recurrent hyperthyroidism after a treatment resulted in hypothyroidism, as expected. Careful discussion with your doctor about risks and benefits of a second (or even third) radioactive iodine treatment vs surgery is needed.

Yes: Assuming you are talking about radioactive iodine, yes. When hyperactive thyroid is given iodine for treatment you hope for normalizing the function. But 25 percent of the time the hyper function can recur and retreatment is necessary.
