If : If a standard drug screen is included with the physical then the marijuana will definitely show up. Nicotine is typically not screened for, however there are tests to check for it; the question is if your employer runs this test or not. Depends: Drugs are rarely looked for in blood tests - the preferred testing is of urine. Doctors usually do not do drug testing for a routine physical, and if they were, they would get urine either instead of or in addition to blood. It is never the wrong time to quit smoking, especially of tobacco, and you need to consider whether the marijuana is a problem as well. Talk to your doctor.
