Tylenol, (acetaminophen) pain drops: After a day or two on antibiotics, he should be more comfortable, unless the drug is not working or there is something else going on. There are topical drops for pain, but these can mask ongoing infection or other problems; tylenol (acetaminophen) should help also. If it has been more than 2 days and he is still miserable, call your doctor as he/she may want another look.

Earache?: A 4 month old with suspected ear problem should be seen by a pediatrician asap. May require antibiotics.
