First : First of all, you must be sure that the pain is coming from your shoulders and not your heart. Have you had an ekg? It concerns me in particular that you have the pain in your arms and your palm. Please see your doctor and be sure that it isn't from your heart. You may have arthritis or bursitis in your shoulder. If so physical therapy or analgesics such as Acetaminophen or Ibuprofen can be helpful, but again please be sure that this isn't pain from your heart.

Eliminate overhead.: If you are doing alot of overhead or above chest level strengthening, you could be prolonging your discomfort. Try avoiding those activities and see if your shoulders feel better.
