The : The term lung "infiltrate" is a non-specific radiographic finding and can represent many things other than infection. If pneumonia is suspected based on clinical features including abnormal chest x-ray, fever, cough, and purulent sputum then a culture and sensitivty should be obtained prior to the initiation of antibiotics. Additionally, blood cultures are recommended in patients admitted to the hospital for community acquired pneumonia. Obtaining cultures and sensitivities prior to the initiaton of antibiotic treatment for pneumonia ensures that the appropriate antibiotic has been chosen and proper therapy is carried out.

Yes: The only way to get a definitive sample of the presumed stuff in the infiltrate is to stick a needle through your chest wall or stick a tube down your airway and get some through that route.Hacking up a lugie provides a contaminated specimen. Research provides us with guidelines on the best abx choices to make.Some times we just have to pick one and hope for the best.
