When : When blood is transfused, it is given through an iv. The blood is taken from donors, sometimes even a family member, then screened through an intensive process. It is then typically crossmatched to be sure there won't be a reaction with the recipient (the person receiving the blood). There are some religions which prefer not to receive blood donations or refuse blood donation. For those patients, there are alternatives to blood transfusions, so they can still have surgeries in most cases. If a patient does not want a blood donation, it is important for them to communicate that to their doctor.
