Not necessarily: I assume that the patient's kidney insufficiency is due to diabetes. Regardless, the need for Insulin therapy is not dependent on the degree of kidney dysfunction. In fact, Insulin requirements often go down with progressive kidney disease.

No : You would only need Insulin if your body is not making enough. Most diabetics actually make too much insulin, this is called metabolic syndrome. Your doctor may want to check your serum Insulin levels before prescribing insulin.
