Privacy: Your medical history should be kept private. That said, it is important to share all your medical history, including previous plastic surgery, with any physician who is treating you so they have the full information to be able to treat you. You should feel comfortable that your doctors will not judge you based on what medical information you give them.

Should be: There are two kinds of "plastic surgery". Reconstructive and cosmetic/aesthetic surgery. Both of these types of procedures should become part of your medical record as it could affect future treatements you need for medical conditions. Your doctors should know about all your prior procedures as it can give good insight into future surgical procedures and choices of needed procedures, .

Define "record": Every practitioner is required by their state medical board to document any evaluation, assessment and treatment in a retrievable medical record. They are required to keep this documentation for a specified number of years & send copies to other doctors when requested. Whether or not this would be in another doctors record will depend if you authorized it. Any insurer involved in payment would.
