Yes: Very early in the pregnany, before 8 weeks or so it is possible not to see both babies.

Depends.: Unless your doctor is experienced in transvaginal sonography, early twinning may be missed by casual transabdominal scanning. After 8-10 wweks, most obstetricians should be able to see if you are carrying one or more fetuses.
