Avoid any trauma: Keloids usually occur if there is any trauma to the skin. It can be surgical or traumatic. A keloid is an over production of scar tissue that grows outside the boundaries of a wound. It can be very difficult to control so the best thing to do is to not have any trauma to the skin.

Depends: Many of us have a tendency to form thick, hypertrophic scars. For some, this is a genetic trait that is difficult or impossible to avoid. Early management of a scar--massage, gel sheeting, etc can be helpful but might not be preventive.
