I would suggest: You make an appointment with an oral surgeon for an evaluation / treatment. I'm sure physical therapy will be part of the treatment. In the meanwhile try to limit wide opening of your mouth.

See an oral surgeon: You are in a closed lock and the joint must be reduced (put back into place). If it has been 4 weeks the articulating disc can exhibit changes that can make it almost impossible to reduce it manually. Surgical intervention may be required.

TMJ disk: Sounds like a "closed lock" of the TMJ disk. Requires complex evaluation/treatmrnt. See TMJ expert. Any dentist can be a TMJ expert with the proper training and experience. Most commonly, oral surgeons, prosthodontists, and orofacial pain specialists. Ask your MD, your dentist and your dental society for referrals.
