MD, first, then DDS: You need to see your md to have the tingling arm evaluated. The headache and teeth pain may come from you clenching your teeth due to the arm pain. After the numbness is evaluated, see a TMJ dentist or bite dentist to have any habits of clenching addressed.

SCM pain pattern: Look to the sternocleidomastoid muscle for trigger points and perhaps the scalenes. Your pain pattern mimics that muscle trigger point map. Of course other causes may overlap that one.
