Yes: Perforation of the stomach or any part of the gastrointestinal tract is a very serious condition. It can be fatal if aggressive treatment including emergency surgery is not carried out.

Very serious: Yes you can die. Stomach acid will leak out and also there would now be an opening to the outside world where bacteria, etc can get into the sterile abdominal area. Additionally, the bleeding from the rupture could also kill you.
