See a dermatologist: It sounds like you have tried a lot of things without results. I think a dermatologist could put together a treatment plan with finesse to help your skin improve. Make sure that they schedule and you attend followup appointments to tweak your progress in the right direction.

Accutane: Recommend Accutane (tretinoin). Very effective for cystic acne. Highly teratogenic (birth defects) so must take birth control at same time. Use for 6months at 1mg/kg dose. Watch for depression/suicidal ideation. Skin will look worse for first 6-8weeks, then improves dramatically.

Details matter: Which birth control? Some birth control makes acne worse, some make it better. How long? Spironolactone dose? Duration? Usually it takes 3 months to appreciate results. Isotretinoin [Accutane] will rescue you if other therapy fails. Usually acne does NOT get worse at first. Typical treatment is 20 weeks. Large studies indicate clearly that it does NOT cause depression and that we waste time lookin
