See a dermatologist: It sounds like you have tried a lot of things without results. I think a dermatologist could put together a treatment plan with finesse to help your skin improve. Make sure that they schedule and you attend followup appointments to tweak your progress in the right direction.

Accutane: Recommend Accutane (tretinoin). Very effective for cystic acne. Highly teratogenic (birth defects) so must take birth control at same time. Use for 6months at 1mg/kg dose. Watch for depression/suicidal ideation. Skin will look worse for first 6-8weeks, then improves dramatically.

Details matter: Which birth control? Some birth control makes acne worse, some make it better. How long? Spironolactone dose? Duration? Usually it takes 3 months to appreciate results.
