The X-ray suggests..: Pulmonary tuberculosis but does not say anything about whether the patient has active infection. Symptoms suggesting activity are fever, cough, night sweats and weight loss. Your friend needs to see a doctor and have sputum tested for the tubercle bacillus.

Old tb?: Sounds like old TB but your friend should see a pulmonologist to know for sure.

Not clear why was the chest xray done andwhat were his symptoms. If he had any symptoms and other than this old TB changes , there was no other abnormal lung shadow ,than this shadow is to be considered accounting for symptoms. Reactivation TB,scar cancer are possible from old TB .As Dr.Lessnau,MD recommended get an appt . with pulmonologist for evaluation and reassurance.
