Go to emergency room: Unless you have a history of migraines that consistently present in this manner these symptoms need to be considered a possible stroke and evaluated in the emergency room. If you do not present to the emergency within the critical time window you lose the opportunity for optimal medical intervention.

See your doctor soon: You have a very high cholesterol, which may have a genetic cause (perhaps familial hypercholesterolemia). It is hard to know what is causing your symptoms, but ischemic heart disease can certainly cause some of these. You need to see your doctor about this right away, probably a cardiologist. If the symptoms are non-cardiac (such as a migraine), you still need to have your cholesterol treated.

Any dentist can be a TMJ expert with the proper training and experience. Most commonly, oral surgeons, prosthodontists, and orofacial pain specialists. Ask your MD, your dentist and your dental society for referrals.
