Possibly: Ruptured appendix can cause peritonitis which can lead to pelvic adhesions. Pelvic adhesions can then possibly cause blocked tubes. I would have an hsg performed to make sure tubes are open.

Yes, but uncommonly: A ruptured appendix can cause infection of surrounding structures. This can lead not only to blocked tubes, and also to scarring of the tubes and ovaries. Any of these conditions can cause infertility, but it is uncommon today that the infection reaches this stage.
