Possibly yes: A ruptured viscus like the appendix can be messy stuff, and cause considerable fibrosis. It's possible such scarring could negatively impact your fertility, but a fertility specialist is best equipped to address this and other possiblities. Don't give up trying to conceive after just 4 months however.

Possibly, keep tryin: You are correct in suggesting that your remote ruptured appy can cause pelvic adhesions and tubal factor infertility. On the other hand, infertility is diagnosed at your age after >12 months of regular (i.e. 3 times weekly) vaginal intercourse without successful conception. If that period has lapsed, see a reproductive endocrinologist infertility specialist with your partner to get resolution!
