Shoulder pain: If not significantly better in 4-6 weeks it should be looked into more with x-rays, ultrasound, or mri.

It depends: If this is truly just a subscap strain and not a partial tear, then healing will likely be in the 6-12 weeks range. Of course, healing rate depends on your age, ability to rest the injured tendon, the effectiveness of rehab and a few other factors such as smoking status, etc.
