We worry about : Pain and entrapment of bowel contents that can lead to strangulation. Having esld complicates things. Discuss with your hematologist and liver surgeon.

It's Complicated: End-stage liver disease is often accompanied by ascites, which is a bad combination with hernias. Surgery is reserved for the rare hernia where the risk of not fixing it is higher than the high risk of surgery (incarcerated hernias, severe pain).

Yes: Hernias that are left untreated can become worse in terms of enlarging size, or worsening pain. They can also cause abdominal contents to become trapped and strangulated, requiring emergency life saving surgery. This last bit is unlikely, but also unpredictable.
